Summary

Surveyors found that the facility failed to provide a homelike environment for its residents, as evidenced by multiple observations of unclean and damaged equipment and furnishings in resident rooms. Specific findings included tube feeding poles with dried brownish substances, broken window blinds, privacy curtains with brown stains or hanging off tracks, and walls with brown splashes and holes.
