Summary

The facility failed to provide timely and appropriate notice of discharge to a resident and the Ombudsman. Record review showed that a resident, admitted in May 2003 with diagnoses including type one diabetes mellitus, celiac disease, hypokalemia, degenerative disease of the nervous system, and long-term insulin use, was cognitively intact according to the most recent MDS assessment. Despite this, there was no documentation that a discharge notice was given to the resident or the Ombudsman prior to the resident's discharge in December 2025. Staff interviews confirmed that no discharge notice had been provided, as the resident was agreeable to leave, and that the Ombudsman had not been notified as of the interview date.
