Summary

The facility failed to report an allegation of misappropriation of property for a resident to the State agency, as required by policy. Despite this, the administrator confirmed that no self-reported incident had been completed for the missing items, and there was no documentation of efforts to remedy the situation or determine whether the items were misappropriated or misplaced. The deficiency was identified during a complaint investigation and was a repeat finding from a previous survey.
