Summary

The facility failed to maintain and post daily nurse staffing sheets for five specific days within a 62-day review period. Record review showed that nurse staffing information was missing for two days in July and three days in August. Interviews with the Nursing Staff Scheduler revealed that while she prepared and provided weekend staffing postings to the weekend supervisor, she did not receive the completed weekend staffing sheets back and was unaware of their whereabouts. The Nursing Staff Scheduler also could not explain the absence of weekday staffing sheets.
