Summary

The facility failed to provide required documentation or notification regarding the bed hold policy to residents or their responsible parties during hospital transfers for two of four residents reviewed. For one resident, there were two separate hospital transfers, and in both instances, neither the resident nor the responsible party received information about the bed hold policy.
