Summary

The facility failed to post the required daily nurse staffing sheet for one of four days reviewed during the recertification survey. During an initial tour, surveyors observed that the staffing sheet posted at the South nursing station was outdated, displaying information from two days prior. A subsequent review of the facility's daily posted nursing sheets for the surrounding days revealed no other issues. Staff interviews indicated that the process for posting staffing information involved the Scheduler preparing and placing the sheets in a staffing book, with the expectation that the night shift nurse would post the new sheet around midnight. For weekends, multiple days' sheets were prepared in advance and left for the night shift nurse to post accordingly. Interviews with the involved nursing staff revealed that the nurse assigned to post the sheet on one night was not accustomed to working that shift and forgot to update the posting due to a busy morning. The nurse scheduled for the following night also failed to update the sheet, stating she was interrupted to assist with care and forgot to complete the task. Both the DON and the Administrator confirmed that posting the staffing sheet was a routine night shift duty and that there had not been previous issues with compliance. The deficiency was limited to a single day when the required nurse staffing information was not posted as expected.
