Failure to Maintain Resident Dignity

Brickyard Healthcare - Golden Rule Care CenterRichmond, Indiana Survey Completed on 09-09-2025

Summary

The facility failed to treat a resident with respect and dignity by allowing inappropriate conversation about her between staff members. Resident 61 had diagnoses including bipolar disorder, insomnia, and anxiety disorder, and her care plan noted behavioral symptoms including attempts at inappropriate conversations with staff. She reported that a housekeeper told her a CNA said she did not like her, although the resident did not hear or see the CNA make the statement herself. Housekeeper 11 stated that CNA 12 told her in the hallway, outside Resident 61's open door, that she did not like Resident 61 and could not stand her, while making obscene gestures. Housekeeper 11 also admitted she made the same gesture back and later went into the resident's room, where the resident asked what had been said. The housekeeper did not tell the resident what was said and did not report the incident at the time. The ED later confirmed he spoke with the resident about the conversation and found the housekeeper's account inconsistent, but the report documents the inappropriate staff conversation involving the resident and the failure to maintain dignity.
