Summary

Surveyors observed that the facility failed to maintain a safe, functional, sanitary, and comfortable environment in multiple areas, including the 100, 200, and 300 Halls, the common area near the nurse station, and the lobby. Specifically, there was a delay in repairing damaged ceiling tiles and a noticeable accumulation of dust-like material on ceiling vents.
