Summary

The facility failed to maintain an effective pest control program, as evidenced by multiple observations and staff and resident reports of cockroach infestations in several resident rooms. During an observation, live and dead cockroaches were found in a resident's room, including on the floor, trash bin, nightstand, and inside a nightstand drawer containing food items.
