Summary

The facility failed to ensure that residents were free from accident hazards and did not provide adequate supervision to prevent accidents during transportation. Three residents experienced incidents related to unsafe transport practices. One resident, who was dependent on staff for transfers and used a wheelchair, reported falling to the floor of the facility van when the driver swerved to avoid another vehicle. The resident was not secured with a seatbelt, slid out of the wheelchair, and was assisted by dialysis clinic staff.
