Summary
The facility failed to ensure that the care plan for Resident #48 was accurate and up to date, as required by regulations. Record review and staff interviews revealed that Resident #48 exhibited behaviors such as wandering into other residents' rooms, drinking from their cups, eating their food, and retrieving ice with bare hands, as well as public urination and using other residents' items.
