Summary

The facility failed to document and promptly address a resident's grievance regarding a room change request. The resident reported experiencing difficulty sleeping due to a noisy roommate and stated that she had communicated her concerns and request for a room change to both a nurse and the DON in late August. Despite this, a review of the facility's Concern/Grievance logs for the relevant period showed no record of the resident's complaint.
