Summary

The facility failed to monitor and implement a resident’s fluid restriction for a resident admitted with end stage renal disease, muscle weakness, and diabetes. The resident was able to make needs known and stated during interview that they had a fluid restriction but were not sure how much, and that facility staff did not monitor their fluid intake. Observation showed the resident had a measured cup with 500 ml of fluid and an empty coffee mug on the overbed table. The provider’s order, dated 11/04/2025, directed a 2000 ml daily fluid restriction with dietary providing 360 ml at breakfast, lunch, and dinner, and nursing providing 460 ml during the day and 460 ml at night. Staff interviews showed CNAs did not know the resident was on a fluid restriction, one CNA stated they learned of restrictions from nurses and did not know this resident had one, and another CNA stated they would look at the care directive or red tag but was unaware of this resident’s restriction. The RCM/LPN stated nurses monitor fluid restrictions and there were no signs or indicators to show a resident was on one, while the DON stated fluid restrictions should be in the care plan and there should be a sign indicating staff were aware of them. Observation later showed the resident in their room with a 500 ml metered cup of water and four plastic bottles of sparkling water on a shelf.
