Incomplete care plans and missing care conference

Summary

The facility failed to develop and/or implement comprehensive resident-centered care plans for three residents reviewed for care planning. Resident 45 was admitted with dysphagia and moderate cognitive impairment; the MDS dated 06/16/2025 documented that the resident required set-up or clean-up assistance with eating and was on a mechanically altered diet. On 08/20/2025, the resident was observed eating breakfast in the dining room with staff assistance, and the EHR showed a diet order for a regular diet with easy-to-chew texture, mildly thick consistency, small dining for meals, and a scoop plate with all meals. However, the care plan dated 08/20/2025 did not include feeding assistance, and the RD confirmed that this need was not in the care plan and should have been. Resident 70 was admitted with left-sided hemiplegia and was cognitively intact; observations and assessment showed the resident had contracture of the left upper extremity, with the left arm across the chest and the fist closed, and the resident unable to independently open the hand or stretch the arm. The DNS/RN stated the care plan was expected to include a focus on contractures, but the care plan had no focus addressing the left upper extremity contracture. Resident 17, who was severely cognitively impaired and had IBS, reported having a lot of diarrhea, yet the care plan dated 08/19/2025 did not include IBS, and the RN/Resident Care Manager confirmed it should have been included. The facility also failed to ensure a care conference occurred for Resident 15 after admission; the guardian said they did not believe a care conference/care planning meeting had occurred, the EHR showed no documentation of attempts to schedule or hold an initial care conference, and the DNS confirmed there was no documentation that the resident or representative had been offered or participated in one since admission.
