Summary

PASARR screening for mental disorders or intellectual disabilities was not reviewed accurately for 6 of 8 sampled residents, resulting in Level 1 PASRR forms that did not reflect documented mental health diagnoses and, in several cases, did not indicate that a Level 2 PASRR evaluation was required. Resident 29 had diagnoses of major depressive disorder and bipolar disorder, but the 10/18/2020 Level 1 PASRR did not list depression and did not select the need for a Level 2 evaluation. Resident 1 had bipolar disorder, major depressive disorder, and anxiety disorder, but the 08/07/2021 Level 1 PASRR did not list anxiety and did not indicate a Level 2 evaluation was required. Staff C, Social Services Director, reviewed both records and stated the forms should have been redone. Resident 17’s 07/23/2024 Level I PASRR included a mood disorder diagnosis, but after later diagnoses of psychosis disorder and delusional disorder, no new PASRR was submitted to include those diagnoses. Resident 6 had major depressive disorder and anxiety, but the 03/13/2024 Level I PASRR, completed as a resubmission due to a significant change in condition, did not document anxiety. Resident 58 had major depressive disorder, psychosis, and bipolar disorder, but the 01/18/2022 Level I PASRR included only major depressive disorder and bipolar disorder and omitted psychosis. Resident 8 had major depressive disorder and delusional disorder, but the 05/05/2025 Level I PASRR documented no mental health diagnoses. Staff C stated the correct diagnoses should have been included on the Level I PASRRs and they should have all been resubmitted.
