On 07/28/2025, five residents attended and there was no documentation that resident rights were discussed or copies were provided. On 08/25/2025, six residents attended and three residents received a copy of the Resident [NAME] of Rights, but there was no documentation that the rights were discussed or reviewed with all residents. A poster titled, Know Your Rights, with an attached binder and laminated sheets was observed posted high on the wall next to the dining room, and it was not at a level readable or accessible for residents using wheelchairs. Staff stated residents were notified of their rights on admission and that resident rights should be reviewed and discussed periodically with all residents.

The administrator stated there was no evidence the resident was provided written information regarding resident rights on admission or after, that the facility did not have an admission packet for the resident, and that the packet could not be found.

A blank admission Agreement reviewed during the survey did not have prices for non‑covered services filled in, and the Director of Guest Services explained that when she populated the form with the date, the prices and costs of services would automatically populate. When asked to locate the signed admission Agreement for this resident, the Director of Guest Services searched the EMR and confirmed there was none, stating she did not know what happened but recalled she was the only person in Guest Services at the time and had taken a vacation during that period. When questioned about written notice of costs for services not covered once the resident’s benefits ended, the Director of Guest Services stated that the resident or representative would have had a conversation with the Business Office Manager (BOM) and/or Social Services Director (SSD). In a separate interview, the BOM and SSD stated that written information about room rates and non‑covered costs was contained in the admission Agreement and acknowledged that, for this resident, they had only verbal conversations with the resident and her daughter about costs, with no documentation of written notice.
