Summary
The facility failed to ensure that a physician completed a discharge summary for one resident who was discharged to a senior living community. Clinical record review showed that the resident was admitted to the facility and later discharged, as documented in a nursing note. However, at the time of the survey, there was no evidence in the clinical record that a physician discharge summary had been completed for this resident.
