Summary

The facility failed to demonstrate active involvement of staff and residents in determining staffing needs during its facility-wide assessment. The assessment identified insufficiencies in staffing, training, services, and personnel. Interviews revealed that CNAs and Resident Care Managers were not asked to provide input regarding staffing needs based on resident acuity, and there was no formal process for staff to give feedback to management about staffing.
