Summary

Surveyors identified that the facility failed to maintain a sanitary and homelike environment on both nursing units. Multiple cracks were observed in the basins and around the drains of shared bathroom sinks in several resident rooms. The Maintenance Supervisor acknowledged awareness of the deteriorating condition of some sinks and confirmed that while some had been replaced, others remained in poor condition pending approval for renovation.
