Summary

Surveyors observed multiple deficiencies in the facility's environment, including a lack of maintenance and cleanliness in key resident areas. In the North Unit dining room, the flooring was found to be uneven, warped, and cracked, with a towel placed under an in-wall air conditioning unit. Clean linens were transported through the South Unit in an open laundry basket, exposing them to potential contamination. Overhead air vents in both the North Unit hall and the shared shower room were noted to have a significant build-up of dust, and one vent appeared rusted. The shared shower room in the middle hall, used by residents from both the North and South Units, had three broken floor tiles near the base of the commode, which itself appeared unclean. A small swarm of gnats and flies was observed around the commode, indicating unsanitary conditions.
