Wrong Resident Medication Administration Resulted in Hospitalization

Summary

A resident with severe cognitive impairment and diagnoses including acute and chronic respiratory failure with hypoxia, diabetes, COPD, and schizophrenia received another resident’s morning medications after a medication administration error. The resident’s care plan included interventions for altered respiratory status, use of anticonvulsant medications, and dehydration or potential fluid deficit. The record states that the resident was supposed to receive multiple scheduled 9:00 A.M. medications, including dutasteride, ferrous sulfate, fluoxetine, Miralax, pravastatin, tamsulosin, benztropine, Eliquis, famotidine, fluphenazine, and gabapentin, and that he had already received his prescribed insulin that morning. The error occurred when an RN mistakenly administered medications belonging to another resident who had the same first name and lived nearby on the same hall. After the resident took the medications, the nurse realized she had been viewing the other resident’s EHR. The medications given belonged to a resident whose scheduled regimen included glipizide, Norco, levetiracetam, lisinopril, metformin, propranolol, tramadol, alfuzosin, aspirin, celecoxib, Zyrtec, Celexa, cyclobenzaprine, and famotidine. The nurse reported that she normally verifies the resident’s first and last name and date of birth before medication administration. Following the error, the resident was assessed, vital signs were checked, and the NP ordered transfer to the hospital for evaluation. Hospital documentation stated the resident had received someone else’s oxycodone and Ultram, did not normally take those opioid pain medications, received IV fluids, and was given two doses of Narcan through the IV. A urinary drug screen confirmed opiates in the resident’s system.
