Summary

The facility failed to maintain a clean and hazard-free environment for thirteen out of nineteen residents reviewed for physical plant problems. Observations revealed that multiple resident rooms, including those on the fourth floor, had ceiling tiles in disrepair with a raised black substance resembling mold, as well as dirty sprinkler heads and vents.
