Summary

The facility failed to provide enough CNA staffing to meet resident needs, including timely toileting assistance, call light response, showers, ambulation, and fall-prevention checks. The report states the facility had 53 residents and that staffing on evening shift was often 2 CNAs on each floor, with the DON confirming that the facility staffed based on census and that the evening CNA staffing was increased only after the census rose in the second week of October. The resident’s father stated the mat had never been placed in the room, that the resident had fallen twice the day before, that he stayed with her around the clock because of her fall risk, and that staff did not check on her every 15 minutes as planned. He also stated it usually took more than 15 minutes for staff to answer the call light. Another resident with dementia, parkinsonism, hypotension, and BPH required substantial to maximal assistance with toileting hygiene and had a care plan for assisted toileting every 2 hours while awake. The resident and spouse stated toileting was not timely and call lights sometimes took almost an hour to be answered, and a grievance documented a wait of over 20 minutes for help with toileting. Additional residents and CNAs reported that staffing was not enough on all shifts, that showers were not always completed, that residents were not being walked as planned, and that call lights took a long time to answer when only 2 CNAs were assigned per floor.
