Summary
The facility failed to provide timely and properly completed Advance Beneficiary Notice (ABN) forms to two residents who were reviewed for beneficiary protection notification. One resident, with diagnoses including amputation aftercare and malnutrition, was required to pay out of pocket for care starting on 5/20/25, but did not sign the ABN form until 9 days after services began. Another resident, with a history of left femur fracture and pulmonary fibrosis, signed the ABN form, but the form was not dated, making it unclear when the notification was provided.
