Summary
The facility failed to ensure that controlled medications were properly tracked and secured, as evidenced by incomplete documentation on narcotic accountability sheets for one of two medication carts reviewed. During audits of the medication cart on the [NAME] Unit, it was observed that the narcotic accountability sheets covering multiple dates were missing the required signatures of two licensed nurses, as per facility protocol, when accepting or releasing the medication cart.
