Summary

Surveyors observed that the facility failed to maintain a sanitary, orderly, and comfortable environment in 10 of 60 resident rooms. Specific findings included the accumulation of residue on telephones in several occupied rooms, loose or broken air conditioning covers, and black and white residue on the slats and inside vents of wall-mounted air conditioning units. Additionally, a resident bathroom was found to be missing an emergency call light cord, and a bedside commode in another room had a rusted part near the toilet seat. These issues were identified through direct observation during the survey. Interviews with facility staff, including the Director of Maintenance and the Housekeeping Supervisor, confirmed that staff are expected to clean resident phones daily, but residue was still present. The Director of Maintenance was unaware of the missing emergency call light cord and acknowledged that some air conditioning covers were not properly secured due to broken clips. Photographic evidence was obtained to document the presence of residue in air conditioning vents in multiple rooms.
