Failure to Initiate Contact Isolation for Resident with Shingles

Summary

The facility failed to implement its infection control policies when Resident 1 was diagnosed with shingles and was not placed on contact isolation precautions for approximately 16 hours. Resident 1 had been admitted to the facility on 6/20/2023 and readmitted on a later date, with diagnoses including polyneuropathy, essential hypertension, low back pain, and legal blindness. The resident’s MDS dated 9/25/2025 indicated cognition was intact and that the resident required varying levels of assistance with eating, oral hygiene, toileting hygiene, and personal hygiene. On 11/22/2025 at 7:48 a.m., a COC assessment documented redness and rashes at the left rib cage with occasional discomfort, and the physician and resident’s representative were notified. RN 1 stated she texted the physician using the facility cellphone and sent a photograph of the skin condition at 7:22 a.m.; the physician responded at 11:27 a.m. with a diagnosis of shingles. The Infection Preventionist stated she was in the facility that day but was not notified of the diagnosis until the morning of 11/23/2025, and she stated that if she had known on 11/22/2025, she would have immediately transferred the resident to another room and placed him on contact isolation. Multiple nurses acknowledged that the resident was not placed on contact isolation or moved to another room when the shingles diagnosis was known. RN 3 stated she was aware of the new diagnosis but did not place the resident on contact isolation or facilitate a room change. RN 4 stated he was informed toward the end of the shift and did not implement a room change, and RN 5 stated she was informed around 11:00 p.m. but did not initiate the room change because the roommate was sleeping. The DON stated the resident should have been isolated and transferred as soon as the physician informed the licensed nurse of the shingles diagnosis on 11/22/2025.
