Summary
The facility failed to ensure that Minimum Data Set (MDS) assessments were transmitted to the State within the required 7-day timeframe following completion for one of fifteen sampled residents. Medical record review showed that a resident expired at the facility, and although the discharge assessment was completed, it was not submitted as required.
