Missing COVID-19 Vaccine Documentation for Multiple Residents

Menomonee Falls Health ServicesMenomonee Falls, Wisconsin Survey Completed on 08-04-2025

Summary

The facility did not ensure medical records contained documentation related to COVID-19 immunizations for 3 of 5 residents reviewed for immunizations: R5, R4, and R2. The facility policy dated 9/17/2024 states residents and staff are to be educated and offered the COVID-19 vaccine, and the resident medical record is to include documentation of education, each dose administered, or the reason the vaccine was not received. R5 was admitted with diagnoses including malignant neoplasm of the rectum, bipolar disorder, and PTSD. The MDS documented a BIMS score of 15, indicating R5 was cognitively intact, with no depressive symptoms and no behaviors during the look-back period. Survey review of the electronic medical record did not locate evidence that R5 was offered, received, or declined the COVID-19 immunization. An LPN reviewed the record after being asked about the issue, and the President of Success-D stated there was no documentation of risk versus benefit regarding R5’s COVID-19 vaccination. R4 was admitted with hypertension, diabetes mellitus, and end stage renal disease, and the immunization record showed a COVID-19 vaccine on 4/20/21, but survey review did not locate documentation that R4 was offered or refused the vaccine after admission. R2 was admitted with Cognitive Communication Deficit, Chronic Kidney Disease, and Weakness, and survey review likewise did not locate documentation that R2 was offered or refused the COVID-19 vaccine after admission. In both cases, the LPN/Infection Preventionist reviewed the concern, and the President of Success-D stated the facility had no documentation showing the residents were offered or refused the vaccine after admission.
