Summary
The facility failed to ensure that five Certified Nursing Assistants (CNAs) received the required 12 hours of annual training, including education in dementia care and abuse prevention. During a survey, the Director of Nursing (DON) was unable to provide evidence that these CNAs, all of whom had been employed for at least several months, had completed the mandated training. The Nursing Home Administrator (NHA) also confirmed that there was no documentation available to show that the annual training requirement had been met for these staff members.
