Documentation supporting the sending of invitations to residents and their representatives was not available, and staff interviews confirmed that there was no system in place to track or document these invitations. Another resident expressed dissatisfaction with not being involved in the care planning process and stated that he had not been invited to any care plan meetings. Staff interviews revealed confusion regarding responsibility for sending invitations, with the Director of Social Services stating it was the Clinical Reimbursement Coordinator's responsibility, while the Coordinator claimed to send invitations but could not provide any documentation to verify this. Both residents had the capacity to participate in their care planning, but the facility did not provide evidence that they or their representatives were afforded this opportunity.
