The staff interviewed on-site agreed that the residents appeared disheveled and that the rooms were not clean or homelike. The regional corporate nurse and the facility administrator both confirmed that the residents should have been showered, dressed in clean clothes, and that the rooms should have been cleaned. The observations and staff confirmations indicate a failure to provide a safe, clean, and comfortable environment for the residents, as required.
