Summary

The facility failed to ensure that Pre-admission Screening and Resident Review (PASRR) assessments were updated to reflect changes in a resident's mental health status. According to facility policy, staff are required to review and update PASRR screenings periodically and as needed, based on changes in a resident's condition.
