The resident reported not being offered activities and only going outside once with a visitor. Record reviews showed a lack of documentation regarding the resident's participation or refusal of activities, contrary to facility policy and staff expectations. The Activities Director acknowledged that the resident was not attending group activities and had not received one-on-one visits as planned. The Administrator confirmed that activities should have been offered and documented for each instance of participation or refusal, but this was not done for the resident in question.
