Summary

Surveyors observed multiple deficiencies in the facility's maintenance and housekeeping practices across three units and the main dining room. Over several days, a large carpet stain was noted in the hallway across from a resident room, and staff interviews confirmed that housekeeping was expected to address such stains promptly. Additionally, missing and broken window blinds were observed at the ends of two hallways, with broken blinds found on the floor. Staff acknowledged the need for repairs and replacements to maintain a clean and functional environment for residents.
