The resident was admitted with diagnoses including Parkinson's Disease, depression, and anxiety, and was noted to be cognitively impaired but able to make needs known. The PASARR Level 1 form incorrectly designated the resident as having a dementia diagnosis, which was not present at the time of admission, and this error was not identified by the facility. Staff interviews confirmed that the resident did not have a dementia diagnosis and that a PASARR Level 1 with a request for a Level 2 evaluation was not initiated, despite the presence of mental health issues such as anxiety and depression.
