Summary

The facility failed to ensure that the daily nurse staffing form was accurately completed and posted with the current census, actual number of staff, and hours worked for each shift on four reviewed days. Observations revealed that the posted staffing information at both the main entrance and nurse's station consistently displayed data from the previous day rather than the current day.
