Facility staff failed to ensure that effective communication training was completed for five direct care staff members, including two CNAs, an RN, an LPN, and the director of rehab. Document review and staff interviews revealed that there was no evidence these employees had received the required training on effective communication, as mandated by facility policy, which specifies that direct care staff must complete at least twelve hours of annual training, including effective communication.
