Summary

Facility staff failed to maintain all mechanical and electrical equipment in safe operating condition, specifically in the kitchen area. Observations revealed that meals were being served on Styrofoam trays due to the dishwasher and oven being out of service. The Dietary Manager confirmed that both pieces of equipment were not working but was unable to specify how long they had been inoperable. Documentation from the repair technician indicated that the dishwasher had not been washing since at least late March, with a significant roach infestation discovered inside the unit, which delayed repairs until extermination could be completed. The oven was also found to be nonfunctional due to a faulty fan motor, with repairs and parts replacement occurring after a delay. Facility records showed that work orders and repair requests were made, but the timeline for repairs extended beyond the facility's policy for timely repairs, especially when outside resources or parts were required. The maintenance director only contacted the repair company for urgent service after surveyors arrived. The facility's documentation and staff interviews confirmed that the equipment remained out of service for an extended period, impacting the kitchen's ability to operate safely and in accordance with policy.
