Summary

The facility failed to maintain a sanitary, orderly, and comfortable environment due to ongoing issues with flooring adhesive seeping from under the tiles, resulting in dirty and sticky floors. Observations over several days revealed that the glue was oozing out, causing the floors to collect hair and debris, which also accumulated on the wheels of medication carts.
