Summary

The facility failed to provide necessary assistance with activities of daily living (ADLs), specifically personal hygiene and bathing, for a resident with severe cognitive impairment. The resident, who was admitted with a BIMS score of 0 indicating severe cognitive impairment, required partial to moderate assistance with bathing and transfers.
