On 05/14/25, an observation throughout the facility revealed that this information was not posted in any location. During an interview, the Administrator confirmed that the postings were not present and admitted to not paying attention to what was displayed on the facility walls. The Administrator was unable to provide a reason for the absence of the required signage and acknowledged the importance of such postings for residents to know how to report concerns about staff.
