Summary
Facility staff failed to maintain a safe, clean, comfortable, and homelike environment on one of three nursing units, specifically the third floor. Multiple observations on different days revealed a persistent strong odor of urine throughout the South and [NAME] unit hallways, even when no residents were receiving incontinence care at the time.
