Summary

The facility failed to make ongoing efforts to resolve grievances presented by the Resident Council/Food Committee regarding food temperatures. Meeting minutes from June and July 2025 documented that residents repeatedly expressed concerns about meals, specifically that coffee and French fries were not served at desired temperatures, and that overall food temperatures were unsatisfactory. Multiple resident interviews confirmed that meals, including those served in the main dining room, were not consistently at preferred temperatures, and one resident reported that her food was sometimes not edible.
