Summary

Facility staff failed to provide necessary housekeeping and maintenance services to ensure a clean, safe, and sanitary environment for a resident. Multiple observations over two consecutive days revealed that the resident's fitted bed sheet had large tan stains and several long streaks of a dark red substance. These stains were consistently present on the bed sheet during each observation, indicating that the bedding had not been changed despite being visibly soiled.
