The facility failed to ensure that food and drink were served at safe, palatable, and appetizing temperatures, as required by federal and state regulations. Multiple residents submitted concern forms over several weeks, reporting issues such as cold food, poor taste, unpalatable meals, small portion sizes, and late meal service. Interviews with residents and the Certified Dietary Manager confirmed ongoing dissatisfaction with food temperature, palatability, and timeliness, despite previous assurances from dietary staff that concerns would be addressed. The facility's posted meal schedule was not followed, resulting in further delays and contributing to the serving of food at unsafe and unappetizing temperatures. These findings were corroborated by direct observation, resident interviews, and review of concern forms, establishing a pattern of noncompliance with dietary service regulations.
