The facility failed to ensure that a resident identified as high risk for wandering and elopement had a person-centered care plan individualized to their specific needs. However, the interventions listed in the care plan were generic, such as administering medication and initiating psychiatric or psychological evaluations, without any individualized or person-centered strategies tailored to the resident's unique needs or behaviors. Facility policies require that care plans be reviewed and revised upon status changes and that residents at risk for wandering or elopement receive care in accordance with person-centered planning. Despite these policies, the care plan for this resident did not include specific interventions or goals addressing the resident's individual risk factors or behaviors related to wandering. This deficiency was confirmed by the Director of Nursing during an interview, who acknowledged the lack of individualized, person-centered interventions in the care plan for the resident at high risk for wandering or elopement.
