Summary

The facility failed to provide a clean, safe, and homelike environment on one of its nursing units, affecting 12 out of 17 residents. Observations included unclean and sticky floors in common areas and resident rooms, unmade beds, and the presence of refuse such as empty cups, food, and soiled gloves in various locations.
