Summary

The facility failed to designate a qualified individual onsite to be responsible for implementing infection prevention and control programs and activities during a specified period. Review of the Infection Control Committee meeting records for the third quarter revealed that the facility did not provide signatures of attendees for the July, August, and September meetings.
