Summary

The facility failed to ensure sufficient supplies, specifically bariatric sheets and towels, were available to meet the needs of residents on one of two floors reviewed. Multiple staff members, including CNAs and the Maintenance Director, reported ongoing shortages of bariatric linens and towels since February 2025.
